Overseas Australian Passport Application
(ADULT – age 18 years or older)
To lodge this application, you must bring the following to your interview:

your most recent Australian passport (if you have one)

your completed application form
-
proof of Australian citizenship
-
your original Australian birth certificate; OR your original Australian citizenship certificate

if you were born in Australia on/after 20 August 1986, you also need to provide one of the following:
-
your previous Australian passport issued on/after 1/1/2000;

one parent's original Australian birth certificate;
-
one parent's original Australian citizenship certificate
-
the certificate must confirm that citizenship was acquired before your date of birth
-
-
one parent's Australian passport
-
issued on/after 20/8/1986 and valid at the time of your birth
-
-
an original Australian citizenship certificate of your own
-
-
if you were born overseas, you need to provide one of the following:
-
your original foreign birth certificate
-
if the document is not in English, please refer to Translation of Foreign Documents Checklist

your valid foreign passport
-
-
Approved ID Combination, including valid government-issued photo ID, and proof of your current residential address.

If you are changing your name, you will need an approved name change document.

Two (2) new, compliant passport photos (less than 6 months old).
-
do not attach your photos using paper clips, staples, tape or glue/adhesive.

if you are replacing a lost, stolen or damaged passport, you will need to provide a written statement using General Declaration Form B11 that explains how, when and where the incident occured.

Credit card (Visa or MasterCard) or debit card for payment
Also, please ensure:
-
your guarantor has properly completed Section 11
-
your guarantor is eligible and is not related to you by birth/marriage or residing in the same household
-
your guarantor has written on the back of your photo “This is a true photo of YOUR FULL NAME” and signed underneath the statement.
-
there are no alterations (no whiteout) on the guarantor or applicant signatures/dates at Sections 11, 17, 18
-
you use the Australian date format (DD/MM/YYYY) at all relevant sections, including Sections 11 and 17.
-
section 11 and 17 are dated within 6 months of your interview date.
-
all signatures at Section 17 and Section 18 are fully contained within the white signature boxes.

digital signatures will not be accepted
Passport photos are checked for size and quality. Tip: use a magnifying glass to ensure the photos are not grainy, blurry, pixelated or striped. There should be no flash reflection, shadows or ‘red-eye’.
Your application form does not have to be printed on Australian A4 size paper. However please ensure your application is correctly sized to either letter or legal size paper ensuring no details have been cut off from the top or the bottom of the pages.
